Occupational Therapy Assistant Application for Admission

The Occupational Therapy Assistant program is a competitive entry program and positions are limited. 

All applicants must complete pre-requisite courses before applying. Please refer to the OTA Admission Requirements page and Steps to Apply page before you complete this application.
