Steps to Apply to the Vocational Nursing Program


PLEASE FOLLOW THE STEPS BELOW TO APPLY TO THE PROGRAM.

 

STEP 1

BECOME ELIGIBLE TO APPLY FOR LVN ADMISSION

The Vocational Nursing program admits students each fall semester.

Enrollment is limited and program requirements must be met in order to be eligible.

New students and students returning after a year must

Submit Other College Transcripts

You must submit official transcripts from any previous college you have attended to ºÚÁÏÉç. This can be done via eloctronic transfer or hand delivered in a sealed official envelope. 

TEAS Test 

Must meet a minimum overall score of 58% to be eligible to apply to the LVN program. Scores will be used in the ranking process.

Virtual Information Sessions  Available October 1, 2024

 

STEP 2

APPLY FOR ADMISSION TO THE LVN PROGRAM

The following items will be submitted electronically, as instructed, after viewing the information session.

When you are ready to apply, return to the virtual information session and navigate to the submission page.

You will need to upload the following documents: 

Application, TEAS Report, Degree Audit, Work History &/or Certification documentation, Proof of HepB vaccination, Signed Criminal History form

PROCEDURE

DETAILS

Documentation of healthcare certification or work history

Healthcare certification or work history and/or documentation of certifications: CNA, CMA, Paramedic, EMT, Phlebotomist, or equivalent certification may be submitted. (Must be current with expiration date in order to receive ranking points).

Documentation of HepB vaccines

Documention of at least two (2) HepB vaccines (Total series of 3) for admission. Titer showing immunity will be accepted.Documentation of at least one (1) HepB vaccine if receiving Heplisav-B (Total series of 2)

Signed Criminal History Packet

Form in Criminal History Packet - will be available at the end of the information session.  Simply read, sign and resubmit.

ºÚÁÏÉç Degree Plan

Submit copy of degree audit; located in Self Service under Unofficial Transcript. 

Transcripts

If you have courses from any other college not reflected on your degree audit, submit the unofficial transcript with your application. 

TEAS

Submit a copy of your official TEAS score report with your application. 

Submit Vocational Nursing Application

Submit the Vocational Nursing Application by the application deadline.  See Step 3 for documentation required to return with application packet.

 

STEP 3

ACCEPTANCE TO LVN PROGRAM - NEXT STEPS

An applicant will receive an email with their acceptance status ie: full accept, alternate or denial.

If you receive full accept or alternate, start gathering the documents below.

DOCUMENTATION TYPE

DETAILS

Documentation of CPR 

Documentation of American Heart Association Healthcare Provider CPR (BLS) Must be in effect through August 31 of the graduating year.

It is the student's responsibility to keep current with CPR. If CPR expires during the program, the student will not be allowed to attend clinical and will be in danger of being dismissed from the Vocational Nursing program.

Complete Physical Examination

Upon acceptance, a ºÚÁÏÉç Health Evaluation form will be sent to students and must be completed by a healthcare professional.

Mandatory drug screen

Students will receive information to register and pay for a drug screen that will be scheduled by the vocational nursing program. 

Record Keeping agency

Students will be required to register and pay for a designated 3rd party company where all documents will be submitted and tracked. 

Registration required by first week of June.

Vocational Nursing Orientation 

Required to attend a mandatory orientation on the date scheduled by the VN program. Proof of all vaccinations must be submitted by orientation date, usually held in July.   

Clearance Outcome from Texas Board of Nursing

Names of accepted students will be submitted to the Texas Board of Nursing. Upon notification by the Texas Board of Nursing including completion of the FBI check and finger printing, all students must provide a copy of their clearance by the TBON (blue card) or "Clearance Outcome Letter" to the NC Vocational Nursing program no later than the date designated in the acceptance packet. 

Failure to provide proof of clearance to the VN department by the first day of class may result in the student being withdrawn and the student may not attend the program.

Declaratory Order

Students that receive notification of a positive criminal history must submit a declaratory order as instructed by the Texas Board of Nursing. Gather any court documents that may be required. 

 

IMMUNIZATION TYPE

IMMUNIZATION REQUIREMENT

Titers showing immunity are accepted if applicable

Measles / Mumps / Rubella (MMR) or Positive Antibody Titer

  • Two immunizations for Measles/Mumps/Rubella (MMR), or

  • a Positive Antibody Titer showing immunity to all three.

TDAP

  • Proof of one dose of TDAP

    • If no record then, you will need to receive TDAP.

    • If TDAP was administered > 10 years ago, you will need TD booster. (TD booster is required every ten years thereafter)

TB

There are currently two FDA approved blood tests - the QuantiFERON TB Gold Plus (QFT Plus) and the T-Spot TB test (T-spot).  

If there is a history of positive TB screen, completed treatment or a negative chest xray within 2 years must be submitted along with a questionairre. Please see student handbook for details. 

Varicella

Must have proof of two vaccines or provide appropriate laboratory serum titer levels. Documentation of history of the disease is not sufficient.

Hepatitis B**For more information, see HEPATITIS B NOTIFICATION below 

Documention of at least two (2) HepB vaccines (Total series of 3) for admission. Titer showing immunity will be accepted.

Documentation of at least one (1) HepB vaccine if receiving heplisav-B (Total series of 2)

HepB series can take 6 months so start NOW!

Influzena Vaccine

Required to show proof of receipt of the current fall season influenza vaccine prior to the beginning of the fall clinical rotation.  

Meningitis

All students within the age requirement must have proof of the vaccine per College policy on immunizations for ºÚÁÏÉç. (21 years old or younger). 

COVID-19

ºÚÁÏÉç does not require a COVID-19 vaccination, however, Clinical sites may require COVID-19 vaccination in order to enter the facility.  Being able to participate in clinical rotations is vital to completion of the Vocational Nursing program.
